Hello guys, I have three concerns regarding TPS. It would be a great favor if any of you could explain.

(1) On the form I-765, #16 regarding the eligibility category, do we need to choose (a)(12) or (c)(19)?

(2) On the form I-821, # 11 "country of residence", you need to write USA since you are residing in USA, right?

(3) If your last entry in the US was in Atlanta. on the form I-821, # 24 a, "US port of entry (if any)", do you write Atlanta or leave it blank? Because in # 24 b, "City or Town", you write Atlanta.

(1) For #16 (a) (12)
(2) #11 USA
(3) don't know yet
I have few questions as well if anybody could help--
Form I765
1) I received EAD from school before so now in the section "I am applying for" is this a "permission" or "renewal"?
2) On #11, again school has applied for employment authorization for OPT so is my answer "Yes" or "No" here.
Form I821
1) For 25.b. Do I mention my cpt expiration date here? or on I94?
2) Do I need to fill Part 6 at all if I am filling it myself?
Lastly being a student do I need to send all previous I-20s along with this application?

I am almost in your situation.
This is what I did;
I 765:
(1) Permission, because you are not renewing under the same category. You are requesting for permission of EAD under TPS.
(2) Yes. (You already did it, though it was different category)

I-821:
(1) CPT expiration date
(2) Not at all.

For no 3: dont leave it blank, put Atlanta as port if entry if that in fact is the one, city is also the pirt of entry
Sometime name if port of entry is different from the name of the city.....but in your case, its the same

#11 on I-821 country of residence : Nepal or USA. Please make sure
